Case Management
My Sister's Keeper of the Triangle, Inc. provides case management that includes the following core components:
- Assessment, Care Planning & Resource Development
- Advocacy, Service Coordination & Monitoring of Services
- Reassessment
The goal of case management is to facilitate the recipient’s medical, social and educational needs. It is designed to empower clients through education, referrals and facilitating access to care, and assisting clients to gain the tools needed to be their own advocates and navigate the health care and social services systems to improve health outcomes.
The case management process requires the consent and active participation of the client in decision-making and supports a client’s right to privacy, confidentiality, self-determination, dignity, respect, nondiscrimination and compassionate, non-judgmental care from a culturally competent provider and quality case management services.
